Allina Home and Community Services (HCH) is the trusted provider and expert resource in delivering patient-centered care and services for patients and families needing rehabilitation, advanced-illness or end-of-life care. With headquarters located in St. Paul, Minn., services are provided in 19 Minnesota counties, 24 hours a day, seven days a week.

Responsibilities:

- Provides primary health care in collaboration with physician to residents in long term care (LTC) and assisted living (AL) settings.
- Provides clinical management and on site visits in collaboration with physician to patients in transitional care settings (TCU).
- Provides timely documentation in electronic record.
- Communicates with patients, families, nursing facility staff, clinic staff hospital staff and community based services to provide appropriate care.


Qualifications:


- Bachelor's of Science degree in Nursing.
- Master of Arts degree in Nursing.
- Adult Geriatric or Family Nurse Practitioner certification.
- 5 years RN in a clinical setting.
- Clear and effective communication.
- Excellent clinical skills.
- Ability to set priorities.
- Ethical decision making.
- Highly organized.
- Able to make sound decisions under stress.


Pay, Benefits, & Work Schedule


- 40 hours per week.
- Day shifts; Monday - Friday.
- One call 1-2 evenings / month.
- Weekend call 3 - 4 times per year.
